Carotid artery endarterectomy was performed in a patient with a rare anomaly of the carotid bifurcation. The external carotid artery was absent, and 3 branches originated from the internal carotid artery. Two other branches emerged from the area of transition of common to internal carotid artery. The arteriographic and surgical implications are briefly discussed.
