Patient empowerment, knowledge and an integrated care team are the keys to successful chronic disease management. The Diabetic Association — now known as Diabetes UK — has always advocated patient-centred care and has provided a platform for the patient, the healthcare professional and scientist to meet. From the beginning of the Charity's history, patients and professionals have worked together to promote the present day Mission: `to improve the lives of people with diabetes and to work towards a future without diabetes'; and to set people free from the restrictions of diabetes, provide the highest quality care and information for all.
