Abstract
This article examines a National Health Service (NHS) secure service provision for people with learning disabilities and complex health care needs in Cambridgeshire, England. It explains how this service operates, the types of referrals it received during the first 5 years after opening and the problems encountered for the service, for the NHS as provider and for the primary care Trust as purchaser. It concludes on the future direction and need to refocus the service to meet the local health care needs of this population, including greater partnership and joint collaboration with social service departments as key stakeholders, and other agencies to plan and develop services to move individuals from a secure hospital environment into the community.
