A range of therapeutic groups, held over a period of three years and facilitated by a community adolescent mental health service, is described. Factors in the success and failure of these groups are identified. Taking these factors into account, the following theory is constructed: adolescents in groups embody institutional metaphors by enacting metaphoric transformations. Implications for clinical practice are discussed.
