We report a case of a fatigue fracture of a Gamma nail in a patient with an unstable oblique subtrochanteric fracture, together with scanning electron microscopy analysis of the fracture surface. In contrast to other published cases, the fracture was in the part of the nail distal to the insertion point of the lag screw.
