Abstract
A new operation for treating snapping of the hip due to the vertical fibres of the iliotibial tract catching on the greater trochanter is described. A “V”-shaped proximally based flap of fascia lata that divides the vertical fibres of the iliotibial is rotated anteriorly. It preserves the function of the iliotibial tract by maintaining its integrity and length. It preserves the function of the gluteus maximus, and covers the greater trochanter with soft tissue; the muscle fibres of the gluteus maximus. The results of the operation in ten patients with eleven affected hips are presented. There was one minor wound infection, otherwise all patients were cured of pain and snapping. All patients had a negative Trendelenburg test at a minimum of two years after the operation.
