An 82-year-old Greek male with left neovascular age-related macular degeneration presented for his 10-weekly aflibercept injection (pre-filled syringes). He previously received bevacizumab and aflibercept (vials) and had uncomplicated left cataract surgery.
He was incidentally found to have asymptomatic left eye intravitreal silicone oil droplets. Fundus imaging, OCT (Figure 1(a)) and B-scan (Figure 1(b)) confirmed the largest droplet anterior to the macula, measuring 3.01 mm (estimated volume 0.014 ml). Intraocular pressure was 15 mmHg. Review of prior wide-field fundus autofluorescence (Figure 1(c)) and pseudo-colour imaging (Figure 1(d)) confirmed the droplets were present for at least two years.
Silicone oil droplets occur in up to 70% of patients post intravitreal injections. Most are asymptomatic, although some cause floaters.1,2 The largest droplet to date measured 1.3 mm (estimated volume 0.001 ml) 3 ; ours is fourteenfold larger. Intervention is generally not required unless floaters are bothersome, in which case vitrectomy has been suggested.2,3
