Abstract
The timeliness of a patient's admission to hospice services remains a topic of both dialogue and debate, but when mortality rates in a home care department exceeded the rates reported in the hospice, the leadership recognized a defining moment and responded. Acknowledging that an agency that provided both home care and hospice services was obligated to ensure that those services were effectively coordinated, the organization embarked on a journey to improve its performance and learned much about itself along the way.
