Abstract
This article reviews the design, implementation, and evaluation of a disease management program for home health care patients with the diagnosis of congestive heart failure (CHF), Rationale for the program, programming considerations, and outcome measurements specific for a home care program are discussed. Use of technology to assess patients is reviewed. Program evaluation, areas of concern, and reimbursement issues are examined.
