Abstract
A process called "automatic assessment" was designed to expedite the discharge planning proc ess through earlier identification of potential home care patients. The physician and referral nurse identified high-risk populations and outlined generic standing orders so that patients meeting these criteria could be identified earlier and assessed for home health and hospice services. This process contributed to more efficient and effective discharge planning. Key words: discharge planning, automatic assessment
