Abstract
A community health nursing perspective of home health care management and practice is hereby conceptualized within the context of the Zimbabwean health care system and primary health care approach. Home health care, known as community home-based care, is an integral component of community health nursing practice. A framework is developed to depict the main goal of community health nursing practice as enhancing client empowerment in independent functioning through cognitive and skill development, cumulating in client participation as a health outcome. Client participation is conceptualized as the extent of self-initiated engagement in activities contributing to health and well-being. Community health nursing practice incorporates family members, significant others, and village health workers in enhancing client participation as a health outcome in the home scenario.
