Chronic conditions are the major cause of illness, disability, and death in the United States. The elderly have more disabling conditions to service in the home care industry. Medicare+Choice HMO plans must maximize opportunities inherent in the managed care model of health care delivery to improve the quality of chronic care by integrating home health services, tracking patients, and measuring outcomes. Complex case management is care coordination provided in a systematic standardized way across the continuum of care. With Prospective Payment Systems, it is timely for case managers and home health nurses to work closely together.
