Abstract
Improved quality of life for clients in the community is the goal of home care, but a strategy is needed to bring that goal to the forefront of service and to advance its focus in clinical practice. A model for advancing a quality-of-life focus in home care, developed as part of a research intervention, has been utilized in several large home care settings. The model addresses the planning and documentation of services, case conferences, and administrative issues. The intervention is directed at nurses involved in care planning.
