Abstract
This article deals with how an informed network of linked services directed by one case manager was able to achieve successful steps toward interrupting the vicious cycle of adolescent pregnancy. It is the story of how a community of caregivers worked together to prevent duplication of services in an era in which erosion of services is occurring. A case study is presented illustrating the value of a one-to-one relationship between a professional caregiver and a teen. Efforts coordinated at various levels in the community provided services, support, and direction to the welfare of this adolescent mother and her child.
