Recognition of the "masked presentation" ofgeriatric depression confronts the nurse with a challenge. Two major obstacles to diagnosing are making the differential di- agnosis ofphysical versus psychiatric causes and depression versus dementia. The pharmacokinetics in the elderly are examined with an overview of the drug metabolism of antidepressants. The use of tricyclic antidepressants and the newer agents, including bupropion and the selective serotonin reuptake inhibitors, are reviewed as part of the psychopharmacologic treatment options for geriatric depression. Last, dosing strategies andfactors affecting compliance in the elderly are outlined.
