Isolated gastric pneumatosis (air in the stomach wall) is a rare condition in neonates. The diagnosis is mainly made on X-ray abdomen. The aetiology is not very clear but can be due to underlying inflammation and sepsis or upper gastric tract obstruction. Early recognition and prompt management (keeping nil orally, orogastric tube on free drainage, antibiotics) are the key to good prognosis.
