Abstract
This article addresses the nuts and bolts of drug dispensing and administrations, and highlights common types of medication errors, misadventures in drug administration, responsible for injury and deaths of institutionalized patients. Case reports and analyses of errors involving pharmacists and nurses are presented, with recommendations for the reader to incorporate into their daily practice. Pharmacist readers are encouraged to share this information, and the risk management approach to avoidance of medication errors with nurses and physicians with whom they practice.
