Winchester District Memorial Hospital has maintained near-zero alternative level of care census days for three consecutive years by focusing on partnerships, staff/physician engagement, and maintaining a circle of care with patients and families. The strategies and challenges faced revisiting internal discharge processes and linking directly to their local community through change management techniques are discussed.
