The purpose of this paper is to discuss the concept of performing simultaneous orthognathic and facial cosmetic surgery. At times, this will necessitate rigid internal fixation of the maxilla, mandible, or both. Also, the endotracheal tube may have to be moved from a nasal port to an oral port should a rhinoplasty be involved. The combination of procedures has been performed over the last six years with no apparent adverse sequelae.
