Scalp reduction continues to be an effective treatment of crown vertex alopecia. Success with this technique depends upon proper design and planning, good surgical technique, and proper patient selection. The modified-S pattern has become our most frequently used pattern and we describe its proper design and planning. We also discuss two scalp extension devices that can be used adjunctively for maximizing the amount of scalp removed per procedure.
