Abstract
Home care of hip-fractured individuals, especially the elderly, requires extensive teaching and discharge planning by professionals in the acute care setting. Discharge planning, including a thorough assessment of patients' needs while hospitalized and a specific instructional plan for home care, is expected to facilitate recovery from major hip surgery. This study validates the need for health care professionals to intensify their efforts to provide a comprehensive care plan for these patients and their families.
