Abstract
The Patient-Centered Medical Home (PCMH) represents a promising approach to improving U.S. health care delivery. Yet overemphasis on a “one-size-fits-all” PCMH model could alienate specific patient populations such as older adults and result in PCMH-type care that does not meet its potential for all patient groups. This article suggests that better aligning the standard PCMH model to the needs, preferences, and care realities of older adults could increase long-term success of the model. The field of gerontology has an important role to play in achieving this alignment.
