Four male patients with severe personality disturbance are described in whom surgery on their genital or perineal areas is followed or preceded by increased psychiatric symptomatology. In one case where adequate historical detail was available, the psycho dynamics of the case, including the unconscious meaning of surgery to the patient, are formulated. Suggestions are made on how to identify and manage such patients.
