The reliability and importance of diagnostic ultrasound and urodynamics in benign prostatic hyperplasia is emphasised, while pointing out the limits of medical treatment. Surgery is still the golden standard, despite a high percentage of patients who are “unsatisfied” even though the obstruction has been removed. These cases always present hyperreflexia and alterations of detrusor normal anatomy.
