The Life Script Questionnaire (slightly modified), published in McCormick, Paul, Guide for Use of a Life Script Questionnaire, (Transactional Pubs, 1971, Berkeley, California) is the instrument used in gathering information from which script analysis is made.

Ratings on Work, Conduct, and Attitude are part of the Texas Department of Corrections Point Incentive Program. The custodial supervisor of each inmate rates him in the three categories each quarter. Ratings are accomplished by awarding points. The highest total of points possible is 100; the lowest, zero.
