We report four people with human immunodeficiency virus (HIV) infection and mania whose manic symptoms improved when given the atypical antipsychotic agent remoxipride as an adjunct, and in whom traditional neuroleptics were not well tolerated due to marked extrapyramidal side effects. General issues surrounding choice of pharmacological agent in HIV-related mania are discussed.
