Abstract
Objective: To present the case of a rare ocular side effect induced by topiramate use.
Method: This is a case of a 43-year-old transgender man who presented with unilateral ptosis of the right eye. Ptosis was noted after 4 years of topiramate therapy, prescribed as prophylaxis against migraines.
Results: While some adverse vision and eye effects associated with the use of topiramate have been documented, eyelid ptosis has only been reported in 0.14% of patients using the drug. Thus, topiramate-induced ptosis is a very unusual finding.
Conclusion: Experience with this case has demonstrated that immediate discontinuation of the drug can restore eyelid function within 1 week.
