We present a retrospective review of histopathologically confirmed cases of Xanthogranulomatous pyelonephritis (XGP), which is a rare condition associated with high morbidity and mortality. Simple nephrectomy, though technically not simple, remains the definitive treatment. It is essential pre-operatively to diagnose or have a high index of suspicion of XGP to ensure the best outcome for these patients.
