Streptococcus faecalis is a rare cause of bacterial meningitis and carries a poor prognosis. A source or underlying disease process is almost invariably found. We present details of a patient in whom no precursor was found. Diagnosis was delayed because Gram staining of the CSF did not permit differentiation from Streptococcus pneumoniae. The institution of correct antimicrobial therapy was therefore delayed.
