A case of carcinoid syndrome is reported in which wheeze was the predominant complaint. Studies indicate the presence of abnormalities of both the kinin-forming system and the patient's indole metabolism. Nevertheless, provocation of airways obstruction by intravenous adrenaline did not produce any elevation of circulating levels of kinin, 5-HT or prostaglandin-like activity. The formation of kinin and its disposal was examined. A significant rise in urinary free histamine was obtained.
