A 13-day-old infant with severe unilateral pulmonary interstitial emphysema was managed with continuous mechanical ventilation via right bronchial intubation as an alternative to lobectomy or conservative therapy. Radiographic improvement was observed after 20 hours of selective bronchial intubation. The endotracheal tube was removed after 68 hours; the chest roentgenogram showed complete resolution of the pulmonary interstitial emphysema by the 18th day. Transient right upper lobe atelectasis occurred.
