Abstract
A method for the identification of human-induced equipment failures in complex systems is presented. This method requires a description of the tasks to be performed, the determination that the tasks can be performed in the time available and finally a specification of task criticality. The probability of error occurrence and estimation of the effect of potential errors are also major parts of the analysis. Efforts for failure reduction are concentrated on errors most likely to occur and to affect the system negatively.
