Abstract
The University HealthSystem Consortium (UHC) Pharmacy Council Medication Management/Quality Improvement Committee has revised their template for drugs being researched for formulary inclusion. The new template incorporates controls aimed at reducing preventable patient injuries by assessing the safety risks of medications before they are added to an institution's drug formulary. Implementing this proactive strategy will improve the ability of Pharmacy and Therapeutics Committees to balance the benefits and risks of adding new drugs to the formulary. Key elements for the successful implementation of the UHC safety-focused strategy are discussed.
