Abstract
Today's health care environments often require pharmacists to measure the impact of the services they provide. One of the pharmacist's most important activities has become documentation — to demonstrate what has been done and how it results in outcome improvement and financial containment. This process began in the 1960s and 1970s using paper-based forms and manual tabulation. Over the years, various electronic media have been used to aid the documentation process. This article discusses advances in intervention documentation made possible by the PDA.
