Abstract
Hemi/Sphere is a new programme for the continuing care of stroke patients and their families following discharge from an acute hospital or rehabilitation centre. Located at The Wellesley Hospital in Toronto, this is a multi-disciplinary programme which stroke participants and family members may attend indefinitely. Prior to their admission to Hemi/Sphere the majority of participants had reached a plateau in their rehabilitation. Stabilization and maintenance were not being carried out and a resultant backslide and depression had occurred. The programme is described and a detailed case study is given. It is shown that this final stage of rehabilitation deserves further study and should become an essential part of the Health Care System.
