A case of perilabyrinthitis with an unusual form of nystagmus alerting mechanism is presented. Patients who are found to have a decreased or absent response to caloric or positional testing may, on global digital compression, manifest nystagmus as recorded by ENG. This form of mental alerting is one which has not previously been described, and should be kept in mind when evaluating patients with vestibular problems.
