A 54-year-old woman underwent re placement of a thrombosed Björk-Shi ley aortic valve prosthesis. Post oper ative electrical defibrillation resulted in isolated T wave alternation which progressed to total QRS-T electrical al ternans. This report represents the first description of such a phenomenon.
