Background: We report a case of cervical lipoleiomyoma, a rare, benign neoplasm composed of mature adipose tissue and smooth muscle cells. The majority of published literature involves masses in the uterine body resected via an abdominal or laparoscopic approach. Case: A 50-year-old woman presented for a rapidly growing vaginal mass. On exam, the mass was noted to be cervical in origin. Magnetic resonance imaging was obtained and was consistent with a benign lesion. A transvaginal approach was chosen for excision. Surgery was uncomplicated. Final pathology was consistent with a cervical lipoleiomyoma. Conclusions: This is the first reported case of cervical lipoleiomyoma that is managed surgically with myomectomy via the transvaginal approach. After appropriate imaging, it is feasible to manage this condition with a minimally invasive technique and uterine preservation. (J GYNECOL SURG 31:370)
