Surgical Treatment of Primary Hyperparathyroidism in the Third Trimester

A successful case of adenoma resection in the third trimester with the birth of a normal infant is described, along with a review of the literature on surgical therapy in the third trimester of pregnancy. The mother underwent surgical resection of a right inferior parathyroid adenoma without problems. Currently there is no safe medical treatment to consistently lower serum calcium in patients with hyperparathyroidism. Because of the difficulty in controlling this condition in these patients, surgical therapy is the definitive treatment. Previously, operative treatment of pregnant patients with primary hyperparathyroidism in the third trimester was not recommended. However, patients with symptomatic hypercalcemia or asymptomatic patients with levels >13 mg per 100 ml should be treated with surgery. (J GYNECOL SURG 15:105, 1999)
