ABSTRACT
A retrospective review of cases of vulvar intraepithelial neoplasia (VIN) was made in an attempt to compare the three modalities of treatment: surgical excision, laser ablation, and laser excision. Thirty-eight cases were identified: 94.8% were VIN III, 2.6% VIN II, and 2.6% VIN I. VIN is a condition which frequently recurs after treatment with the majority of recurrences appearing within the first 30 months after initial diagnosis in our study. The recurrence rate in all modalities was frequent, necessitating multiple treatments, and persistent, close follow-up. No difference was found between wide surgical excision and laser excision or ablation, either in the interval to first recurrence or in the percentage of recurrences. The use of laser is advocated in the treatment of VIN as a safe and effective method of controlling the disease, if combined with persistent follow-up.
