Cost Benefit of Effective Diabetes Management in the Managed Care Setting: A Case Study

Abstract

A growing body of evidence suggests that people with diabetes achieve better outcomes if approached with an integrated disease management care plan consisting of clinical guidelines, patient education, access to glucose meters, and information system reporting. The diabetes management program at the Geisinger Health System in Pennsylvania provides a blueprint for success using those guidelines in an integrated care delivery system.
