Abstract
As reengineering continues to change the structure, financing, and methods of healthcare delivery to populations, disease management has become an integral concept in most plans. Different types of disease management programs have been developed, as have different ways of configuring both the program and its financial structure with a health plan, a delivery system, and integrated delivery systems. Disease management can give an organization a competitive advantage as a health plan, as a delivery system, or as a hospital—all very different components in markets. This article describes elements of successful disease management programs and cites types of programs appropriate for different types of organizations.
