Abstract
Congestive heart failure (CHF) is a common and chronic form of heart disease. Disease management offers a new model for more effectively managing patient health care over a continuum. One increasingly popular tool is a dedicated CHF clinic program. Implementation of new programs represents new organizational challenges for health-care providers. Operational planning is a component of the larger strategic planning process. Chances for realizing enhanced operational effectiveness are increased by adhering to a focused developmental process. We review the sequential components of operational planning including directional vision, feasibility studies, pilot studies, and final implementation. This is followed by a case study of a large cardiology group as they went from a developmental concept to a fully implemented clinic.
